Trustee's Deed Required for XTO Engergy

Hello,

Would someone have a sample of a Trustee’s Deed that I need to provide to XTO Energy, Inc., to re-register shares in my name. Per XTO this is what they need, as all other documents are on file, and I do not know where to find a sample that will suffice for XTO.

Provide some more information for better assistance such as what has initiated XTO’s request. The term Trustee and shares create conflicting information.
