The simplest thing to do is to get a document entitled “Affidavit of Fact.” The County Clerk may have one of these or try online.
Have your mother-in-law make a statement showing her name and new address. It will have to be notarized for filing.
The form is pretty simple and does not need to be fancy or long-winded.
