Can anyone help me to understand what this order is about and how to get a replacement copy of said order?

Personally, I am anal about paperwork and redo the whole thing until it is perfect. Sometimes, the only way that I am judged is on my paperwork.
However, feel free to correct, initial and send it in. It will still get the job done.
